ABSTRACT

PURPOSE: The purpose of this study was to construct a clear definition of the concept of clinical supervision among psychiatric mental health nurse practitioners (PMHNPs) in South Korea. METHODS: The hybrid model was used to perform concept analysis on supervision. Results from both the theoretical review of 17 studies and a field study interviewing 12 PMHNPs were included in the final process. RESULTS: The concept of supervision was found to have complex features and to consist of multi-dimensional meaning units, with 4 levels (including 11 attributes and 36 indicators). The dimensions and their 11 attributes are suggested as follows: 1) Learning dimension (intensive practice, clinical knowledge and skills, reflective practice), 2) professional support dimension (supervision function, evaluation and feedback, supervisory relationship, autonomy), 3) institutional dimension (supervision framework, policy of clinical supervision), and 4) consequential dimension (ensure the qualities of nursing care, professional development). CONCLUSION: Based on the results, supervision is defined as: “The provision of guidance and feedback on matters of personal, professional, and educational development in the context of a supervisee's experience of providing safe and appropriate nursing care.” The findings in this study provide helpful insights for understanding supervision and practice development.

ABSTRACT

PURPOSE: The main purpose of this study was to identify the essential meaning of clinical supervision for psychiatric mental health nurse practitioners. METHODS: Data were collected in 2015 through individual in-depth interviews using open-ended questions from 9 women psychiatric mental health nurse practitioners. Each interview lasted about 90 minutes. Verbatim transcripts were analyzed using the Giorgi's phenomenological analysis method. RESULTS: The results of the analysis showed the following four categories: ‘Barren mental health site alone’, ‘Enduring with reflective supervision’, ‘Professional competence enhanced by continuous supervision’, ‘Maintaining a healthy life balance as a psychiatric mental health nurse practitioner’. CONCLUSION: The findings in this study provide helpful insights for understanding the need for clinical supervision in the psychiatric mental health nurse practitioner course. The results also provide support for the psychiatric mental health nurse practitioner course and practice development for psychiatric mental health nurses at the work site.

ABSTRACT

OBJECTIVES: It was the aim to examine the association of the thyroid-related hormones with cognitive function, depression, and subjective memory impairment in community-dwelling elders with questionable dementia. METHODS: The sample consisted of 399 community residents with 'questionable dementia' aged 60 or over in whom serum thyroid-related hormones [thyroid stimulating hormone (TSH) and thyroxine] had been assayed. Cognitive impairment was defined using the Korean version of the Consortium Establish a Registry for Alzheimer's Disease. Depression was diagnosed using the Korean version of Geriatric Depression Scale and subjective memory complaint (SMC) was checked using the subjective memory complaints questionnaire (SMCQ). Age, gender, education, and the presence of apolipoprotein E epsilon4 were included as covariates. RESULTS: There was a significant positive association between verbal fluency test (VFT) score and serum TSH levels (p = 0.01). There was a significant negative association between SMCQ total score and word list memory test (WLMT)(p = 0.002) or word list recall test (WLRT) score (p = 0.013). CONCLUSIONS: Lower serum TSH levels were associated with semantic memory (VFT), and we found that SMC was associated with episodic memory (WLMT and WLRT) in this sample.

ABSTRACT

OBJECTIVE: To investigate the status and basic demand of community based stroke-disabled for rehabilitation program development in an urban area. METHOD: The subjects were 46 residencial stroke disabled over 6 months after disease onset. Two teams (composed of one physiatrist and one nurse, respectively) visited patient's home, and evaluated physical and functional status and surveyed for the status and basic demand of community based rehabilitation. RESULTS: Forty two patients (91.3%) received primary medical care after onset of the stroke, and twenty two patients(47.8%) received rehabilitation therapy. The most common reason for not having received rehabilitation therapy was poor economic state. University hospital was the most common place of their rehabilitative management. Mean score of Modified Barthel Index (MBI) was 60.7. Mean score of Craig Handicap Assessment and Reporting Technique (CHART) was 223 and economic self-sufficiency scale presented the lowest score as 6.5+/-4.6. The desire of patients for rehabilitation and welfare service was rehabilitation management and the desire of subjects for the government and society was warrant for livelihood. CONCLUSION: We concluded that rehabilitation program utilizing common resource such as areal university rehabilitation team and facilities of community health center should be developed.
